「 The most beautiful ladies in the world 」

THE BRIEF HEALTH

5 ways to get better at saying no  / BY ANGELA HAUPT

HERE’S A COMMON MALFUNCTION OCCURS WHEN WELL-INTENTIONED people open their mouths to say no: the word yes tumbles out instead.

We’ve all been there, says Vanessa Bohns, department chair and professor of organizational behavior at Cornell University.  No is a deceptively short, simple word that can trigger several layers of anxiety for the person trying to say it.  “We worry that we’re essentially communicating that we’re not a helpful person; we’re not a nice, kind person; we’re not a team player.”

At the same time, we’re likely stressing over how that no might offend the other person, and what it conveys about our relationship with them.  As Bohns puts it, you might think “it’s telling the person, “your standing with me is not what you thought it was.’ ”

In reality, however, there’s an array of benefits associated with learning to say no. “If you’re saying yes to everything, people are more likely to ask you again and again,” Bohns says.  “You wind up being the person who gets all the asks,  and that can lead to burnout, problems with work-life balance, feeling like you’re being taken advantage of, and a loss of autonomy.”  Plus, an inability to say no could cause priorities such as hobbies, relationships, or projects to suffer.  “Each time we say yes to something, we’re implicitly saying no to something else,” Bohns points out.

Experts recommend these strategies to get better at saying no.

BY ANGELA HAUPT

  1. Be intentional about what you communicate

Bohns advises emphasizing three things: “It’s not because of me, it’s not because of you, and it’s not because of us.”  One way to do that is by thanking people for thinking of you.  Then, follow up with a short explanation: “I wish I could, but I just don’t have the time right now.”  That helps make it clear that your no isn’t a poor reflection of your own character (you would do it): it’s not the other person (you appreciate the ask); and it’s not an indictment of the relationship (it’s simply circumstantial).

  1. Buy time

If you tend to accidentally say yes whenever you’re put on the spot, find ways to buy time. You might say, forexample, “Let me think about it and I’ll get back to you,” Bohns suggests.Or: “Let me check and I’ll respond by email.”That way, you can spend time privately processing the request, making a mindful decision, and if necessary, declining in whatever format is most palatable.

  1. Be matter-of-fact

Delivery is everything when you’re saying no.Avoid over apologizing or otherwise acting as though you’re doing something wrong, advises clinical psychologist Ellen Hendriksen.“If se signal that this is no big deal, and we’d like to help but can’t, that sets the tone for a more neutral interaction, she says. One way to do that, “I counsel students to ask difficult questions in the same tone of voice they would use to order a sandwich.”That strategy can’t be applied to saying no too.You can’t help your sister-in-law’s second cousin move into her new apartment?Treat it as if you’re ordering a tuna sub, hold the mayo.

  1. Adopt the broken-record technique

If someone is applying undue pressure, utilize what Hendriksen describes as the broken-record technique.“It’s sticking to your answer—giving the same answer again and again,” she says.“You don’t have to be soulless about it; you can empathize and be polite. But it’s important not to let your no evolve into a ‘maybe’ or an ‘OK, just this one.’ ”

No " "Maybe " "OK, just this one "に発展させないことが重要です」。
  1. Carry a visual reminder of why your no matters

If you say yes to that committee at work, what will you miss out on? Bohns suggests carrying an inspirational photo that serves as a reminder.“It could be your dog; it could be your kid,” she says.“It could be some hobby that you love.”Put it near your computer or phone, and when you really don’t want to do something—but feel obligated to say yes—it will give you strength.Pick it up and remember: “If I say yes to this, I’m basically saying no to when my daughter asks me if we have some free times to read a book later.”

Climate Is Everything

Towers supporting electricity lines in Brunswick, Maine, this fall

YOU MIGHT BE HARD-PRESSED TO find anyone who actually likes their power company; rates just keep going up even as many utilities drag their feet on allowing renewable-energy projects to connect to the grid.  There’s reason for that: utilities are among the last monopolies allowed in the U.S., so many consumers have no choice about what company sells them their electricity.

But doing anything about your utility’s problem is a herculean undertaking, as Maine environmental groups discovered when they asked voters on Nov. 7 to replace the state’s investor-owned utilities, Central Maine Power and Versant, with a consumer-owned nonprofit utility called Pine Tree Power.  Around 70% of voters voted no, a result that may seem surprising in a state where the two investor-owned utilities are widely reviled; Central Maine Power ranked last among large utilities in the East for consumer satisfaction four years in a row.

Advocates for Pine Tree Power called the vote “the most important climate election in the country this year,” but the utilities spent more than $34 million opposing the referendum, and the election may have come down to money.  And not just campaign spending.  An independent analysis estimated it could cost Maine anywhere from $6 billion to $13 billion to buy out the utilities, which could have led to higher rates initially.

Main isn’t unique in getting cold feet with the idea of replacing hated utility companies; from 2000 to 2019, 60 different U.S. communities considered “municipalization”—essentially buying out an investor-owned utility and making it locally owned.  Just nine actually did it, according to a study by Concentric Electricity Advisors.  One major reason?  Buying all those wires and poles is very expensive.

That doesn’t mean there’s no way to change power companies.  Utilities are regulated by state commissions that ostensibly listen to public input.  Getting consumer advocate elected to these commissions can help.  In December 2022, for example, Louisiana voters replaced an incumbent utility regulator with Davante Lewis, an activist who spoke up for green energy and criticized the commission’s lack of accountability.  Elections matter, even if Main environmentalists may not feel that way right now.

Health final part

As interest in these drugs grows, it’s not so far-fetched to imagine a world where all anyone needs to lose weight is a prescription.  Davisson, the obesity specialist from West Virginia, says about 80% of her patients are on a weight-loss drug, and she feels that anyone who is overweight or obese should consider some form of treatment, since they may develop complications over time even if they’re healthy at the moment.  “Everyone is metabolically healthy,” she says, “until they’re not.”

But other physicians are struggling with their place in this new world.  When Dr. Mara Gordon, a family physician in New Jersey, finished her medical training almost a decade ago, she believed weight loss was a good thing.  But the longer she practiced—and saw how her patients shut down when she talked about weight—and the more studies she read, the more she began to doubt whether weight loss should be an assumed goal.  “I found, increasingly, that it was all downside,” she says.

Gordon minimized weight’s role in her practice, focusing instead on other makers of health—things like insulin resistance, blood pressure, chronic pain, mental health, and quality of life.  Today, though, more and more patients come into her office asking for Ozempic and Wegovy by name.  Often, she says, patients who want to drop pounds are technically overweight but have little medical reason for taking a GLO-1 drug.  Nevertheless, when she looks at the whole picture, the decision becomes more complex.  Her patients’ test results may not signal a problem, but they’re still desperate to lose weight—maybe so they have the energy and mobility to play with their kids, or to improve their body image, or simply fit into a world that prizes thinness.  “If you’re facing hatred and fat phobia on a daily basis, if you can’t do the things you need to do because the chair at your office isn’t the correct size,” a medication like Ozempic may genuinely help, Gordon says.  “I wish we lived in a less superficial society.  But my job is to take care of the patient right in front of me.”

In the end, she often writes the prescription.

OBESITY IS a “common, serious, and costly chronic disease,” according to the U.S. Centers for Disease Control and Prevention (CDC), afflicting more than 40% of U.S. adults and almost 20% of children and putting them a risk of health problems including heart disease, Type 2 diabetes, stroke, and some types of cancer.  If obesity is a disease, it follows logically, that has meant prescribing diet and exercise as plan A.  But in practice, “try as we might, a lot of exercise typically does not result in a significant amount of weight loss,” says Glenn Gaesser, a professor of exercise physiology at Arizona State University.  That’s in part because people get hungrier the more they move, offsetting whatever calories they burn at the gym, and in part because the body tends to get used to its size and work to stay at that set point, Gaesser explains.  Lifestyle changes can work for some people, but many lose only modest amounts of weight or regain the pounds over time—a process known as “weight cycling” that is itself linked to cardiovascular and metabolic health problems.

For a long time, doctors had relatively few options to offer patients for whom diet, and exercise didn’t work—things like metformin, a Type 2 diabetes drug that also causes a modest amount of weight loss, and bariatric surgery, which is effective but few patients opt into.  And then came Ozempic.

Ozempic, Wegovy, and Mounjaro all work by simultaneously slowing digestion and mimicking the appetite-suppressing hormone GLP-1.  (Mounjaro also targets a second type of hormone receptor.)  This double whammy leads to an average 15% to 20% reduction in body weight after about a year.  They don’t work well for everyone, but compared with older meds, “the efficacy of these drugs is remarkable,” Takkouche says.  “The weight loss is undeniable.”  According to data from Novo Nordisk, semaglutide (the genetic name for both Ozempic and Wegovy) also slashes the risk of heart attack and stroke by 20% among overweight or obese adults with heart disease.

After struggling to lose weight through diet and exercise, Justin, a 29-year-old from North Carolina (who asked to use only his first name to protect his privacy), lost about 30 lb. in less than six months on Wegovy.  But as he scaled up his dosage over time, he experienced side effects including acid reflux, nausea, diarrhea, and lethargy.  As much as it pained him, he decided his quality of life was better off the drug.

Since quitting Wegovy in June, Justin has gained back about half the weight he lost, a common outcome for patients who stop using GLP-1 drugs—which many do either because of side effects or cost, since many insurance plans don’t cover weight-loss drugs and out-of-pocket prices can exceed $1,000 a month.  Despite his mixed experience, Justin would still recommend that someone trying to lose weight consider Wegovy, and may someday go back on it himself at a lower dose.  “It made enough of a difference” that it’s tempting to go back, he says.

Many obesity-medicine specialists share Justin’s feelings.  “We have effective tools” for weight loss now, says Dr. Laura Davisson, director of medical weight management at West Virginia University Medicine.  “Why not use them?”

There is one big reason, according to a passionate group of doctors, researchers, and activists: obesity never should have been labeled a disease in the first place—and thus may not need to be treated at all.  “Manipulating weight is not a path to health,” says Ragen Chastain, a patient advocate who co-authored a library of Health at Every Size resources.  “The belief that fewer fat people existing is good—that is weight stigma.”

As Chastain and others like her see it, GLP-1 drugs are not lifesaving anti-obesity medications, but new tools for reinforcing damaging body standards rooted in stigma, not science—while making pharmaceutical companies gobs of money.

HEALTH

It’s unusual for a medication to become a household name; even more uncommon for its branding to become, like Advil, shorthand for an entire class of products; and rarest of all, for it to change not just U.S. medicine, but U.S. culture.  Ozempic has done all three.

Approved in 2017 as a Type 2 diabetes medication, Ozempic has largely made its name—and a fortune for its manufacturer, Novo Nordisk—as a weight-loss aid.  Its runaway success mirrors that of similar medications, including Eli Lilly’s Mounjaro and Wegovy, another Novo Nordisk product and the only one in the trio technically approved for weight loss.  Prescriptions for all of them are flying off the pad at an eye-popping rate: they’re up 300% since early 2020, with more than 9 million written in the U.S. in the last three months of 2022 alone, according to health-care-industry research firm trilliant Health.  Demand is so great that there have been shortages of all three drugs, with some diabetes patients struggling to fill their prescriptions as they compete for limited supplies.

Plenty of physicians (and pharmaceutical executive who stand to get very, very rich) say this frenzy is a good thing, given that roughly three-quarters of U.S. adults qualify as either overweight or obese and are thus, according to leading public health authorities, at risk of a range of serious health complications.  “Obesity is an epidemic, and we urgently need effective treatments,” says Dr. Sahar Takkouche, an obesity and bariatric-medicine specialist at Vanderbilt Health.  But others are uneasy about the age of Ozempic, which can feel like a return to an era when thinners was unquestioningly valued.

Before the Ozempic tsunami, there was growing support for the Association for Size Diversity and Health’s principles of Health at Every Size, which hold that body size is not a measure of health or worth and all people deserve non stigmatizing medical care. The group’s influence contributed to the rise of “weight-neutral” medicine, which sees weight and health as separate, and worked in tandem with the wider body-positivity movement to loosen the diet industry’s viselike grip on American psyches.  As the 2000s progressed, women’s magazines stopped pushing diets quite so hard.  Clothing brands bragged about hiring models larger than a size o.  Evern Weight Watchers rebranded as “wellness” company called WW.

Then Ozempic and its cohort came along, and it turned out lots of people still wanted to be skinny.  In fact, some industry watchers predict the rise of Ozempic and its ilk could spell the end of obesity.  But as these drugs transform both standard medical practice and cultural ideas about weight loss, a contentious debate is simmering beneath the surface: Should we even be treating obesity?

Family is essential. It’s also challenging. Here, tips and reflections on navigating it all

The enduring love of my chosen family

IN AUGUST, I SPENT 10 DAYS VISITING MY FIANCEE ERIN and her son Andy in Maryland.  Erin and I were celebrating our birthdays, and my soon-to-be stepson was about to start third grade.  During my stay, Erin and I told Andy a series of collaborative bedtime stories: Erin would start, and whenever Andy wanted, he’d say “switch,” and I’d jump in to tell the next part of the story.  Together we spun tales of siblings who defeated an oil dragon in a magical realm made of paint, solved puzzles in a world made of food, and more.  On the night before the first day of school, the siblings had escaped from inside a giant cyclops, and as we tucked Andy into bed, we wondered aloud about what both the next story and the school year might hold.

“Thank you for loving me,” Erin said as we walked downstairs to savor the quiet hours between when Andy goes to bed and when we follow suit.

“Easiest thing I’ve ever done.”  My standard response.  A cute little ritual we started shortly after we’d found the courage to say, “I love you,” meant to express both the ease of our love and our gratitude for it.

I met Erin, a fellow activist, in February 2022 amid a moment of crisis for trans people.  Texas’ governor had already ordered the department of family and protective services to investigate families who were suspected of providing gender-affirming care to their transgender children.  Trans youth were not only at risk of losing access to their health care but also of being taken away from their parents and homes.  I took calls from parents who loved Texas, who had lived there for generations, yet were terrified that the state was no longer a safe place to raise the their kids.  Erin and I volunteered to help families raise funds and find safer states to relocate to, and it was in this work we found one another.

Erin and I chatted throughout the spring, then began dating that summer, after my heart skipped just a little too much when she brushed her hair behind her ear on a video call.  Over Thanksgiving, a few weeks after I was elected to the Montana house of representatives, Erin visited Missoula, Mont., for the first time, and a week later, I flew out for what was supposed to be a quick trip to Maryland to visit her and meet her son.  Over the three days, I not only got to see what an amazing mom Erin is, but I also began to form the beginnings of my own relationship with Andy.  On the last night, Andy asked if I could read the bedtime story—my first!—and afterward he told Erin, “Mom, I don’t want Zooey to go.”  Erin and I tucked him into bed, and I changed my flight to 10 days later.

I filled that time with a mix of parks and parkour with Andy, as well as hikes with Erin, all while prepping for the upcoming state legislative session.  Toward the end of my trip, Erin and I were invited to the White House for the signing of the Respect for Marriage Act.  We gathered alongside hundreds of other LGBTQ people in what was a moment of culmination for one aspect of the fight for our rights.  I cried throughout much of the celebration, and as Sam Smith performed a delicate rendition of “Stay With Me,” I learned against Erin and whispered, “I’m going to marry you some day.”

“God,” Erin replied, “I can’t wait.”

Health Matters

By Haley Weiss

IT’S RARE TO HEAR OF AN Alzheimer’s diagnosis being a total shock.  By the time the presence of the disease is confirmed in an older adult, it’s easy to look back and see initial signs of mental decline: a forgotten Thanksgiving recipe, a story from early childhood told one too many times, an extra-confusing grocery bill.  This slight change in functioning that’s just outside the realm of normal, even for an older person, is referred to as mild cognitive impairment, or MCI.  It’s precursor to several conditions that cause dementia, and early detection is critical for the effectiveness of aducanumab, the first-ever Alzheimer’s medication.

With the arrival of treatment, doctors are being forced to reckon with how infrequently MCI is diagnosed.  Recently, a research team at the University of Southern California has suggested that of more than 7 million cases of MCI nationwide, only around 10% are caught.  I talked to Soeren Mattke, one of the lead authors of the study, and he made the following observations:

*Neurological decline can be a major source of shame for older adults, who may go out of their way to compensate for symptoms in order to avoid detection.

*While there are tests for MCI, the shortest takes 15 minutes, which is longer than many physicians have time for during standard checkups.

*Improved detection is particularly important in socioeconomically disadvantaged groups, where risk factors like high blood pressure and cholesterol—both associated with dementia as well as cardiovascular events—are higher.

*Nobody knows more about what’s going on with older adults than their loved ones, who should press doctors for evaluations if they feel it’s warranted.

Mattke says he hopes to do additional research that would allow him to put together risk-scoring algorithms that physicians could use to identify people for testing.  “Brain cells don’t grow back,” he says.  If you feel like you or a loved one might be showing signs of MCI, “don’t let a physician brush it off.”

Gaza part 2

AFTER THE OCT. 7 Hamas attacks that killed more than 1,400 people in Israel and started the war, the Israeli military began a massive bombing campaign.  On Oct. 13, it ordered civilians to depart the northern part of the strip for the south, and on Oct. 27 it sent in ground troops and armored vehicles.  Hamas has fought back above and below ground, from a network of concrete tunnels extending hundreds of miles.

‘We cannot do anything except continue.’

For the estimated 1 million people displaced by fighting, the search for shelter has brought many to makeshift tent cities.  More than 50,000 are packed into the Al-Shifa hospital complex in northern Gaza, says Dr. Ghassan Abu-Sittah, a surgeon.  Mattresses line the floor, kids run around, and a stench hangs in the air.  So many people in such a small space, with inadequate access to hygiene and sanitation, will lead to an outbreak of infectious diseases, Abu-Sittah worries.  Hospitals are struggling to dispose of dead bodies, which pose their own health hazards.  Abu-Sittah has been going to a corner store to buy bottles of vinegar and laundry detergent to clean wounds.  “Every day you make more and more compromises,” he says.

Some two dozen hospitals have been asked to evacuate to the south, according to the WHO, which says doing so would risk patients’ lives.  When Israeli government officials called Al-Awda hospital and told its manager, Dr. Ahmed Mhanna, to evacuate staff and patients, “I refused, of course,” he says.  “Where can I deal with my patients?”

Doctors worry that their facilities will be hit in the bombardment.  On Oct. 30, an Israeli airstrike damaged part of Gaza’s only cancer hospital, the Turkish-Palestinian Friendship Hospital, says its director, Dr. Sobhi Skeik.  “My message is please don’t kill cancer patients,” Skeik says.  On Nov. 1, the WHO said that the hospital had shut down.

Days before that, the Israel Defense Forces presented evidence it said showed Hamas had established a command center in and beneath Al-Shifa hospital.  A Hamas official denied the allegation.  Targeting a hospital would be a war crime, whether or not Hamas is using it to hide in, says Susan Akram, a law professor who directs Boston University’s International Human Rights Clinic.  “Israel has an obligation to protect the entire population in Gaza,” she says.  For its part, Israel notes that using a hospital to hide military equipment or facilities itself a war crime.

Even without a direct attack, the hospitals lack key supplies, which are coming in at a painfully slow pace amid the Israeli siege.  On Oct. 31, the U.S. said that 66 trucks of humanitarian aid were entering Gaza daily, a fraction of the hundreds per day before the war.  Fuel remains a critical issue.  The Israeli military reportedly believes Hamas holds more than 500,000 liters that it could provide to hospitals.  The U.S. says it is pressuring Israel to break its blockade and allow aid in.  President Biden called Nov. 1 for a humanitarian “pause” in the war, but faces criticism for providing military aid to Israel.

Everyone in Gaza has been affected.  “We often focus on the victims of airstrikes,” says Dr. Brenda Kelly, a consultant obstetrician in Oxford, U.K., “but ordinary lives don’t stop.  Women still go into labor.  They still have miscarriages, ectopic pregnancies, preterm births,” Dr. Hatem Edhair, the head of the neonatal intensive-care unit at Nasser Medical Complex in Khan Younis, feats that electricity shutting off will mean the deaths of five babies in his care who are dependent on ventilators.  “If there is no electricity,” he says, “it means the end of their life.”

Al-Awda hospital’s Mhanna, speaking by phone Oct. 23, in southern Gaza, seemed unfazed by the sound of a blast during the interview.  “We are afraid; we are human beings,” Mahanna says, “But we cannot do anything except continue our mission with our patients.”

Gaza's grief

WITH MINUTES OF THE OCT. 31 ISRAELI attack on the Jabalia refugee camp in Gaza, the victims began flooding the Indonesian Hospital a mile away.  Dr. Marwan Sultan, the hospital’s medical director, says that most of the injured and dead were women and children.  Some had deep burns, serious head injuries, or missing limbs, Sultan told TIME four hours after the attack.  There are only 16 intensive care beds in the hospital, which was running dangerously low on fuel, threatening the lives of his patients.  If the electricity goes, says Sultan, “they will die.  They will die.”

The conditions for medical care in Gaza are deteriorating across the besieged 140-sq.-mi. coastal strip.  Surgeons are operating by flashlight and rationing water, anesthesia, and the generator fuel needed to perform surgeries, provide electricity for incubators, and care for kidney-dialysis patients, doctors and health organizations tell TIME.  The roughly two dozen hospital still operating in Gaza are absorbing the patients of the 12 that have closed because of a lack of supplies and the ongoing bombing, says the World Health Organization (WHO).  “Medical teams are on their knees,” says Hisham Mhanna, spokesperson for the International Committee of the Red Cross in Gaza.

All war zones are awful, but Gaza presents a unique hell.  Much of the enclave of 2 million is now a battlefield, with civilians and combatants intermixed, and homes and businesses sitting side by side with military infrastructure.  Nowhere is that reality felt more keenly than at the territory’s hospitals, which have simultaneously become safe havens and potential targets, and where the impact of Israel’s offensive is measured every day in lives—more than 9,000 killed as of Nov. 2, including 135 medical personnel, according to the Palestinian Ministry of Health.

DIED

Suzanne Somers / Entrepreneurial actor

SUZANNE SOMERS WAS never a dumb blonde.  Her breakout character—Chrissy Snow on the ABC sitcom Three’s Company—may have been, but Somers herself was an actor, businesswoman, author, and health and wellness spokesperson.  She died on Oct. 15 at age 76 after a battle with breast cancer.

“I’ve been playing what I think is one of the best dumb blondes that’s ever been done, but I never got any credit,” she told the New York Times in 1980.  “I did it so well that everyone thought I really was a dumb blonde.”

Three’s Company ran from 1977 to 1984, and quickly became one of the country’s most popular shows.  At the start of the show’s fifth season, Somers asked for a pay raise from $30,000 to $150,000 per episode to match the salary of her male co-star.  ABC would offer only a $5,000-per-episode raise, which Somers declined.  The network fired her, and Somers sued for $2 million.  She received only a small fraction of what she asked for.

Somers made one movie, Nothing Personal (1980), and after Company had a string of guest roles and made-for-TV movies before starring in another ABC sitcom, Step by Step, from 1991 to 1998.

She would go on to become a health and diet magnate.  Many of her more than 25 books touched on wellness culture, though her support for bioidentical hormone replacement therapy and alternative cancer treatments have been criticized by the medical community.
